Abstract
SUMMARY
Damage to the integrity of the skin comes from injuries due to trauma and surgery, besides that the skin is depressed for a long time which becomes a compressive wound. The high incidence of pressure sores as well as the impact of the presence of compressive wounds, therefore it is important for nurses to make various efforts to prevent the occurrence of pressure sores in patients by early detection of risk factors for the occurrence of pressure sores. The purpose of this literature study is to identify studies that can be used in early detection of risk factors for compressive injury. Method: Study this literature through searches for scientific publications from 2006-2017 using the Pubmed database, and Google Scholar. Results: the results of literature search from 5 articles obtained. The study identified the assessment and assessment of Pressure Ulcer. The compressive wound assessment tool has been developed to assess the risk of compressive wounds such as the Braden, Gosnell, Norton scale, Waterlow scale. This scale aims to identify the risk of high and low likelihood of the occurrence of pressure ulcers and immediately take precautionary measures to avoid pressure ulcers. Conclusion: Assessment is the first step in the nursing process. Several risk assessment instruments for pressure sores have been made to help nurses. Assessment can be done using the risk assessment instrument for compressive
wounds, so that it becomes the basis for nurses in providing appropriate interventions.
